- Policy: there is one contract number per family. On the My Healthcare Card and in the app, each insured person can find their individual number.
- Name of affiliate: a (1) next to the affiliate’s name indicates that a change has taken place for that person during the invoiced period
- Employer premium: net and gross premium (taxes and NIHDI contributions included) payable by the employer.
- Personal premium: net and gross premium (taxes and NIHDI contributions included) payable by the affiliate. This premium is claimed by AG Insurance from the employer, who recovers the premium from the employee (e.g. via payroll deduction).
- Total: the total of all premiums (net and gross) payable by the employer and the affiliate for all insured persons in this premium statement group
- Total employer + personal premiums: total net premiums payable by the employer and the employee
- Taxes: 9.25% tax due on "Hospitalization" premiums (not applicable for "Outpatient care" and "Dental" premiums). This tax is calculated separately on each individual premium.
- INAMI (= NIHDI) contribution: 10%. The contribution is calculated separately on each individual premium.
- Total: total premium claimed from the employer.
