Please use the "Request for Periodic Surrenders" form to withdraw money from your AG Ascento Free contract on a regular basis or to change the terms of your periodic surrenders for this contract.  

Please complete, sign and return this form to us:

Once we have received this signed form, we will proceed with the payment in accordance with the General Terms and Conditions.

Please use the "Transfer" form for transfers between funds:

• from a fund with a guaranteed interest rate (Branch 21)
• from a fund without a guaranteed interest rate (Branch 23)
